Vetted is seeking a RN - Hospice Palliative Care for a travel job in Yuba City, California .

Must have 2+ years of experience.

This contract pays approximately $13,821/month gross.

Assignment details:

• Contract length: 13 weeks
• Shift length: 8 hour shifts
• Hours per week: 40
• Start Date: ASAP

Job responsibilities:

• Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams.
• Support care coordination activities across providers to promote continuity of care.
• Educate patients on treatment plans, follow-up instructions, and self-management strategies.
• Communicate clinically relevant updates to providers to support timely decision-making.
• Work 40 hours per week with 8-hour shifts.
